How to File a Delta Dental of Virginia Vision Insurance Claim Form

Life happens, and sometimes you just can’t find an in-network eye doctor nearby, or the next available appointment might be weeks away. When that happens, you may need to see an out-of-network provider.

Finding the vision insurance claim form

Delta Dental of Virginia has partnered with VSP® VisionCare to bring you flexible, affordable vision benefits you can trust: DeltaVision®.

To file a vision insurance claim, you’ll need to start by logging into your VSP account. If you have yet to create an account, be sure to do that first by following these steps.

Once you’re logged in:

1. Click on “View Your Benefits”
2. Under “Oops! Did You Go Out of Network?” click “Submit a Claim”
3. Click “Start New Claim” and follow the prompts

Tips for filling out your vision insurance claim form

1. Have your member information ready: Log into your VSP account to find your Patient Member ID, Vision Plan Name, and Group Number.
2. Gather provider information: You’ll need the provider’s name, office or website name (like Warby Parker), address, and phone number.
3. Don’t forget to attach receipts! Make sure your receipts clearly show the services provided, the dates, and the amounts paid.
4. Submit your claim on time: According to EyeMed, most claims must typically be submitted within 12 months of the date of service to receive reimbursement.
5. Keep copies: Before submitting, take photos or scan your claim form and receipts. That way, you’ll have them available to reference in case you need to follow up.
6. Make it easy to read:  If you’re filling out the vision insurance claim form by hand, be sure to use black or blue ink, and make sure all handwriting is clear and legible. This helps avoid processing delays and ensures your claim is reviewed without issue.
